چکیده

further complicated by the fact that unless the PPI molecule is protected, it is subject to degradation in the presence of stomach acid.3 The PPIs (except for Zegerid) are therefore encapsulated or enterically coated to prevent premature activation of the drug in the presence of gastric acid.2,3 Opening the capsule and administering the contents have the potential to leave the granules vulnerable to degradation, premature activation, and decreased efficacy. Information about administering PPIs via enteral feeding tubes is limited. This article discusses the available literature and provides practitioners with suitable methods of administering PPIs via nasogastric, gastrostomy, and jejunostomy tubes. A search of PubMed utilizing the terms omeprazole, lansoprazole, esomeprazole, pantoprazole, rabeprazole, enteral, nasogastric, gastrostomy, and jejunal was conducted. All retrievable in vitro and in vivo studies describing an extemporaneous formulation administered via an enteral tube were included for review.

Short-term proton pump inhibitor treatment may cause hypomagnesaemia in critically ill patients - a pilot study.

Many studies have suggested a link between long-term PPI treatment and hypomagnesaemia, though none of them investigated the short-term exposure in high-risk patients. We sought to investigate this issue in 90 critically ill patients. We assessed serum Mg concentrations, necessity of Mg supplementation, PPI dose, duration of PPI therapy and route of administration. In multiple analysis we found...

Changing how we confirm ngt placement: impact on nutrition delivered

Introduction Confirming the position of Nasogastric tubes (NGT) in critical care is a controversial area. The National Patient Safety Agency (NPSA) guidance [1] advocates pH testing although use is limited in ICU due to continuous enteral feeding and proton pump inhibitor therapy. Chest X-ray is therefore commonly used with trainee doctors often being responsible for confirming tube position. F...
